OPERATIVE REPORT

PREOPERATIVE DIAGNOSIS: RETROPERITONEAL MYXOID LIPOSARCOMA.

POSTOPERATIVE DIAGNOSIS: SAME.

NAME OF OPERATION: INTEROPERATIVE (sic) RADIATION THERAPY


DESCRIPTION OF PROCEDURE: Please see the dictated operative note by Dr. Sam Yoon for details of his operative findings and procedure. In brief, this young man had a localized recurrence of a myxoid liposarcoma around the celiac access (sic). He received 50 Gray of proton radiation therapy prior to surgery. Because he had significant regression of tumor, we thought it might be reasonable to attempt to resect him. Indeed, Dr. Yoon was able to grossly resect tumor from about the celiac access (sic) and preserve the celiac artery. We reviewed the frozen section specimen in the frozen section laboratory and this showed that there was significant treatment effect with at least 99% tumor necrosis. We elected however to deliver additional intraoperative radiation because of the multiple close margins. Hence, we delivered 10 gray of intraoperative radiation therapy to the tumor bed with 6MEV electrons, employing an 8 cm. cone with a 30 degree level with 0.5 cm. Bolus. Dose was prescriped to 90% isodose line. There were no complications of the procedure.

The bolus was removed from the operative field at the end of the interoperative (sic) radiation.
